Empowering healthier hearts
through everyday movement
Healthy Hearts
The challenge
Healthy Hearts launched in 2025 across Lancashire as part of the national We Are Undefeatable movement, with a local focus on improving heart health among people living with, or at risk of, cardiovascular conditions.
Following the huge success of a health campaign across Blackburn in 2024, Hotfoot was again chosen to bring Healthy Hearts to communities across Lancashire, creating a locally resonant and impactful campaign that champions heart health, inclusion, and movement for all.

The strategy
The Healthy Hearts campaign in Lancashire was designed to reach communities experiencing widening health inequalities linked to heart conditions, inspiring people to engage in physical activity that felt relevant and achievable for their lives.
The strategy came to life through a bold, multi-channel activation spanning print and digital social media, using real photography and human, memorable copy to reflect lived experience and build trust. Print materials featured QR codes linking audiences to further information and local support, while enabling meaningful data capture for evaluation.
Key objectives included:
- Inspire inactive people living with, or at risk of, heart conditions to move more for better health and wellbeing.
- Reach communities facing the greatest health inequalities, including deprived areas, minority ethnic groups, older adults, and people with multiple conditions.
- Mobilise partners and influencers by securing local support to amplify the campaign and deliver shared county health priorities.
- Build workforce awareness by equipping healthcare professionals and physical activity providers with clear, confident messaging on movement and heart health

The results
The campaign performed strongly against its stated objectives, demonstrating clear impact across motivation, reach, stakeholder engagement and workforce awareness.
Inspired behaviour change: Encouraged people to increase activity and re-engage with movement.
Reached priority communities: Built strong awareness through a balanced mix of OOH, digital, direct mail and community channels.
Strengthened partnerships: Delivered with Lancashire County Council and the ICB, establishing a new joint approach to public health campaigning.
Raised workforce awareness: Shared across GP and health networks, supporting a shift towards lived-experience storytelling.
Client Feedback
"I’ve worked with numerous agencies over the years and I've become accustomed to meticulously checking that the changes I’ve asked for have been made that’s not the case with Hotfoot – not only are they quick to action requests, but their accuracy is refreshing, eliminating the need for multiple rounds of revisions. Their attention to detail has been a lifesaver for our campaign, which has required rapid turnaround times. Thanks to their organisational skills and ability to manage multiple requests from different colleagues, the campaign has kept on track."
